Provider Communications

You are responsible for reviewing and responding to communications.

We are not responsible for missed communications or outcomes resulting from them.

Subscriptions & Services

Some services may be offered on a recurring subscription basis.

By subscribing, you authorize automatic billing at the agreed intervals.

Your subscription continues until canceled.

You must cancel at least 72 hours before your next billing cycle to avoid renewal.

Cancellations & Fees

Cancellation requests must be submitted in accordance with the cancellation policy.

A $99 administrative fee may apply to eligible refunds

No refunds are issued once medications are processed or services are rendered

Financial Responsibility

All services are cash-pay.

We do not bill insurance, Medicare, or Medicaid.

You are fully responsible for all costs associated with your services.

Payment & Default Policy

You agree to make timely payments.

Late payments may result in:

Late fees

Service suspension

Account termination

Referral to collections

You are responsible for maintaining valid payment methods and sufficient funds.

Collections

Accounts that remain unpaid may be referred to a collection agency.

Additional fees may apply where permitted by law.

Telehealth Services

If applicable, services may be provided via telehealth.

You understand:

Telehealth has limitations and risks

It is not a substitute for in-person care in all cases

You may be required to sign a separate consent form

User Responsibilities

Any disputes will be resolved through binding arbitration.

Proceedings will be conducted remotely unless otherwise agreed

Claims must be filed within one year of the event giving rise to the dispute.

Limitation on Claims

By agreeing to these Terms, you waive certain rights under California law, including unknown claims to the extent permitted.
